Willow Crest Nursing Pavilion, a 116-bed skilled and intermediate care center in Sandwich, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Willow Crest Nursing Pavilion was cited with an A violation in October and fined $25,000.
