a. Resolution R2025-004—A Resolution Authorizing the DCRNC Administrator to Execute the Provider Agreement with Aetna Network Services LLC on behalf of the DeKalb County Rehab and Nursing Center.

As a means to grow the census of the DCRNC, Jordan Healthcare Group has recommended that the DCRNC enter into provider agreements with Insurance Providers. During the month of December 2024, the DCRNC was reimbursed an average of $655.57 per day per Medicare A resident and $403.75 per day per Medicaid resident. The revised Jordan Healthcare Group Business Plan for the DCRNC projects an average daily census of 2.5 Medicare A Replacement Residents.

In June of 2024, the County Board authorized the DCRNC to enter into an Operating Participation Agreement with Illinois Aging Services Network (ILASN). ILASN acts on behalf of their network members to negotiate Medicare A Replacement Insurance contracts. In November of 2024, a number of insurance contacts from ILASN were brought forward for consideration, including a Provider Agreement with Aetna Network Services, LLC. Due to the unfavorable reimbursement rates within the Aetna contract through ILASN, the Health & Human Services Committee voted down the contract and directed staff to negotiate an individual facility contract with Aetna Network Services, LLC.

Resolution R2025-005 authorizes the DCRNC Administrator to execute an individual facility Provider Agreement with Aetna Network Services LLC. This agreement would allow the
